The Traumatic Brain Injury (TBI) Medicaid Waiver Program helps eligible New Yorkers with a traumatic brain injury avoid unnecessary nursing home placement and receive the support they need to live safely and independently in the community.

About the Program

The TBI Waiver Program provides home- and community-based services to individuals living with the effects of a traumatic brain injury. Its purpose is to prevent unnecessary institutionalization and help participants rebuild their independence at their own pace.

All Health Home Care is an approved provider of services under the TBI Waiver. We coordinate individualized, goal-driven care plans designed to support participants throughout their recovery from rebuilding daily living skills to reconnecting with their families and communities.

Services Covered Under the Waiver

Each care plan is developed in collaboration with the participant, their family, healthcare providers, and support team. Services are selected based on the participant’s individual needs, recovery goals, and ability to live safely in the community.

Service Coordination

A dedicated Service Coordinator develops and manages your care plan, helps you access available services, and communicates with the providers involved in your recovery.

Structured Day Program Services

Hands-on training helps participants rebuild the daily living, communication, problem-solving, and self-management skills that may have been affected by a brain injury.

Independent Living Skills Training

Hands-on training helps participants rebuild the daily living, communication, problem-solving, and self-management skills that may have been affected by a brain injury.

Positive Behavioral Interventions and Support

Individualized strategies help participants and their families understand and manage the behavioral, emotional, or cognitive changes that can follow a traumatic brain injury.

Respite Services

Short-term support provides family caregivers with time to rest, attend to personal responsibilities, or recharge while their loved one continues receiving appropriate care.

Community Integration Counseling

Counseling and support help participants adjust to life after a brain injury, rebuild relationships, and become more involved in community activities.

What is the TBI Waiver Program?

Is there a cost to join the program?

Eligible Medicaid recipients generally do not pay out of pocket for approved services provided through the TBI Waiver.
